Well, first you explain to people that if they aren't reformed then they go away for everyone.
Then, you reform them so that even if they're not as great as before, at least they're still there.

Obviously, if the most expensive treatments are just part of every package, then the price of all of the packages goes up so the insurance companies are able to make a profit. Alternatively, the insurance company will cover only a portion of the high expense procedures and the patient will have to pickup the remainder.
